What are the contact form dependencies?

Hi - i used the custom JS package generator:http://support.semicolonweb.com/js-package-generator/

i am not really using a lot of things for my site and don't need it to be slowed down - but i did notice that when i tested the contact form, it works but loads the confirmation message in a blank screen. (you can see it here: http://valliniskippack.com/contact.html)

i did look in the js/plugins.js (original file) and found the jQuery Form Plugin code and copied that into my revised plugins.js - but it is still not working. What other code do i need?
