Using included PHP Code

Dear Sir
recently i used your template to create a website and i need to activate the contact form.

i need the guidelines on how to use the PHP code that included with the template , is this code need to be triggered when the submit button is pressed

if there is a Detailed documentation for how to use your php code please provide
