Subscribe and Contact forms not working

Hi there

I am setting up a one-page site with a simple contact form at the bottom, and 2 subscribe forms (one in a modal and one in the page footer).

The contact form appears to work fine. When submitted the green success notification appears and the form fields are cleared, however, I do not receive the posted information to my email inbox! Can you please take a look at this and let me know how to fix?

As far as the subscribe forms go, when submitted the email address is added to my Mailchimp list successfully, however, the green success notification does not appear, nor does the form field get cleared. To the user it appears to have done nothing. Can you please help to get a notification success working and clear the field? There are two subscribe widgets on the page... one is in a popup modal when a button is clicked, the other is located on the footer.

Thanks for your help. I realize there are dozens of similar posts, but many of them are old posts and I'm not sure the relevance since I am using the latest code build.
