How would you use star ratings in a feedback page and get the star number values returned and associated with the person who has completed the form?

Same with range sliders - if I included them in a form, how would I get the customer info back, associated with that customer?

Hello,
The Latest Form Templates are already included with these Functionalities: http://themes.semicolonweb.com/html/canvas/forms.html . You will simply need to add a name Attribute to the Ratings/Range Slider Input Element. Example:
Make sure that the name Attribute Value of the Rating Input matches the Form's prefix Setting Value.
