star rating 1 not able to select

Question

I'm unable to select star rating 1 on this page. I think it may be due to formatting but I can't seem to figure it out. Here is the link to a temporary page.

We have checked out your Page and the issue is caused due to Customizations. First we would recommend checking the HTML Errors: https://validator.w3.org/nu/?doc=https%3A%2F%2Fwww.balletauditions.org%2Frevnewexpiredaudtest.php%3Finpprogid%3D625 and fix the Unclosed HTML Tags.

Additionally, currently issue caused due to Heavy Customizations does not come under included Support: https://support.semicolonweb.com/support-policy/ as the Feature in question here works perfectly out of the box. We would recommend reviewing your codes further to find out which customization caused the issue.

I removed all of the errors and the validator now displays no errors on page.
Could it be that on the same page I have a section that displays stars with read only but the form is a with stars with selection? In other words, cannot have both types on same page?
