It's possible to have severals modal but with slider like forms-modal-health.html ?

Hello,

Unfortunately, currently, this is not supported. You can only have 1 Modal on the Page. However, you can add a Slider inside the Modal to mimic the behavior or you can consider using a Gallery: http://themes.semicolonweb.com/html/canvas/lightbox.html .
