Form sending only name, email and phone. not the date hours and so on.

Additionally, regarding your Question, it appears that the Form is not configured properly. Make sure that all the Guidelines are followed as in the Documentation > Forms Section and your issue with sending Custom Fields from your Form should be automatically resolved.
