QuickContact form creates a 404 error on send

Hi,

i have a quickcontact form setup and it works (it sends the email to my email address) BUT when i click the submit button i get:
**Not Found

The requested URL /undefined was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.**

and the url at the top is: http://petocklaw.com/undefined

Any ideas why this is happening?

i actually bought more support so i could resolve this - but 2 days later after hearing nothing from the support team - i finally figured it out.
