Please help with form

Dear canvas team.
My son said I should build a form to contact website visitors.
However, I have no idea about PHP etc.
I cleaned the "contact-5.html" file from superfluous things (navigation and footer) and made the entries described on the "https://docs.semicolonweb.com/docs/forms/settings/" page .
Unfortunately I can't get it...
Would you please help me or write what I'm doing wrong?
The website is not yet published, so here is the link and the login:

Attached is the "include" folder and a screenclip
The "contact.html" file is not linked to the other html files

We have tried checking out your Website and the issue appears to be with your Server Configuration as the include/form.php is returning a 500 Internal Server Error. It would be great if you can please provide us with FTP Access so that we can look into this for you and fix it asap. Thanks for your Patience.

Additionally, please make sure that your PHP Version is more than 5.6 or preferably the Latest Version.

If you are still facing further issues, the please consider adding the .customjs Class to the .form-widget Element and then following code at the very top of your include/form.php File:
[ch_pre type="php"]error_reporting(E_ALL);[/ch_pre]

which should display all the possible errors. Please notify us about the Error Message you see, after going through the process above, so that we can provide you with more assistance on this. Thanks for your Patience!

We have fixed the issue. The include/form.php File was incorrectly edited which was causing the issue.
