PHPmailer fails to load resource

I apologize upfront as I'm probably the zillionth posting this issue, but I've checked the forum and stared at code for three days straight and can't seem to fix it.

My form at stroopers.ch/index.html isn't working. It can't find some sort of resource. I have thrown all the resources I have at it, but now I'm ready to kill myself :-) I hope someone would be so kind to take a look and help me out.

I posted this a few days ago, but I don't have a solution yet and I NEED to launch...

We have checked out your Website and the issue appears to be with the File: http://stroopers.ch/include/subscribe.php as it throws a 500 Internal Server Error. Please consider re-uploading this File as this might fix the issue. Thanks for your Patience.

Really Apologize about the Delays! It would be great if you can please provide us with a New Live URL just with the Form so that we can fix this up for you asap. We would love to get your Website up and running as soon as possible. Thanks for your Patience.

Additionally, can you please confirm if you have added your Mailchimp API Key and List ID in the include/subscribe.php File as this might also have caused the issue.
