Need help to use "contact-5.html" as template

Question

Copy the file to my local dev directory, the "CLICK HERE TO SEND AN EMAIL" does not work, no pop up form. It works if I try it in the original "HTML" directory.

Also to get Google map working, how can I get my own key?

Thanks for your timely response. For the pop up form issue, when I copy the relevant files to the LIVE URL location, somehow it works, it appears that I must have messed up something in the development folder. I am able to open the form now, after filling in the form and click the "Send Message" button, how do I get the message send to my own address?
