Need help getting a button form modal on the header

Hello,

I am trying to find a work around and need help with a modal. I need to replace the shopping cart and search from my header and replace it with a button form modal that displays a Contact Form and submits. I have tried several ways and can't seem to get it right. Is it possible to get this done? If so, please let me know the best way.

I am basically trying to get the Link Modal with a working form inside to replace the shopping and search portion of that same header.

This Template is already included in the Package/HTML Folder. Please consider using the header-login-modal.html Template.
