Form not working

Hi, everytime I've already changed the settings in my form.php file but it doesnt work.

Hello, We have checked out your Codes and there are extra spaces in your SMTP Settings (see attached) that could be causing this issue. i also will add an upload button for multiple documents upload, ajax will work fine too?
