I have set the Contact Form up according to directions and it seems to work fine, but I am not getting a form submission to my email. Please help. Here's the working site address: http://websiteprod.com/working/contact.html

Hello,
This appears to be an issue with the Servers rather than the Codes. Please make sure that the Servers are not blocking the PHP mail() else you will need to use the SMTP Function. Additionally, please also check the SPAM Folders as the Emails might get delivered there.
Moreover, do make sure that the From Email used in the include/form.php File is from the same Domain you are hosting the Files on (except SMTP).
