We have checked out your Contact Form and there appears to be some PHP Syntax Error in your include/sendemail.php File: http://www.mirrortap.com/include/sendemail.php . Fixing this error should automatically fix the issue.

Hello,

This appears to be an Issue with your Customized Codes. Please consider editing your File from scratch using the Default Codes that came with the Package to setup your Form's PHP by referring to the Documentation > Widgets > Contact Forms Section. This should automatically fix the issue.
