hi my contact us form http://amzgemz.com/contact.html doesnt work . i did all necessary changes in it also in form.php and still something doenst work. Please help . Im attaching both pages too for recored.

i removed most of the code that i didnt need it , i dont know how that may affect the function of the form.

Modifying the include/form.php File will affect the default functionality of the Form as the issues you have mentioned are caused due to Customization. Please consider starting with the unmodified version of the default file and if you are still facing issues with that, do let us know and we will surely look into the possible issues and fix them for you.
