Hello,
The form is not working, I entered in my information to test it and the submit button doesn't work to even test it.
Also, I have put in the address on the contact-7.html form and for some reason, the red map pointer isn't showing up on the address.

What can I do to resolve this?

Can you please use the Canvas's Form Codes so that we can troubleshoot this at your end and provide you with an appropriate solution.
The Map Marker is not working because it has not been uploaded to your Web Servers: http://javonalyasiri.com/images/icons/map-icon-red.png . Please upload the images/icons/map-icon-red.png File to your Servers and this issue will be automatically resolved.
