I cannot seem to get the contact for to work. It just keeps spinning. include/form.php. I tried changing in the .html page - contact.html - <div class "form-widget"> every way to put in customjs. How should the code work? Every way I try I get a page that comes up saying "Phase error..........includes/form.php line 15

There are PHP errors in the http://www.kentuckyonejobs.com/include/form.php File probably after you have made Customizations to them. Fixing them should fix the issue automatically.

There appears to be an Error on Line 15 of your include/form.php File which is why your Forms are not working. Please make sure that you have properly edited the Files using a Text Editor and it is also recommended to edit the File again to add your Email using the Default File from the Package which should fix this issue.

According to the issue you have been facing it appears that the issue is with the Version of your PHP. The Latest Version of PHPMailers uses PHP5.6 and above, so you will need to update the PHP on your Servers in order for the Forms on your Website to work.

I downloaded everything again on themeforest and replaced the file. It still show version 5.5. Right now everything just sits and spins. I cannot seem to get the email form to work. You do not need to update the Template. You will need to Update the PHP on your Servers so that the PHPMailer can work properly. Please contact your Web Hosting Provider regarding this and ask them to Update the PHP to the Latest Version on their Servers which is Hosting your Website.
