Hello,
As of yesterday (5-17) our contact forms stopped working. Specifically on (https://accesspharmaceutical.com/Apply%20Page.html). I have checked for HTML errors and checked our servers as well. Tried with a few different browsers as well.
Is there any way you guys can check it out and see what can be going on ?
Any help is deeply appreciated.

We have checked out your Website and this is a Server Issue. The File: https://accesspharmaceutical.com/include/form.php is not executable and instead it is getting downloaded in the Browser which is why the Form is not getting processed. You will need to make the PHP File executable to make the Form Work.

So I went ahead and changed the file permissions (see attached) to no avail, should I change it to something else?

Yes, this is a Server Issue and unfortunately, changing the permissions will not help. There is some Server Configuration which is preventing the PHP File from getting executed. Only your Web Hosting Company can help you with this.
