Contact form is not working

i have uploaded form.php and phpmailer folder , but when called i get error 403

This is definitely an issue with the Server Configurations as the PHP must be supported on your Servers and executable. For this issue, you will need to first contact your Web Hosting Provider and get this File working and then if you have any further issues, we will be more than happy to help.

hello,
now the form works fine but attachment is not showing as a file in the email sent , also the form alerts are shown in form.php not within the page itself
