Hello,
We have checked out your Website and your include/form.php File is returning the Error: SMTP connect() failed. This error is related to the Servers and either your Authentication Details are incorrect or your Server is blocking the SMTP from sending emails. You will need to contact your Web Hosting Provider in this case so that they can help you fix this issue.
