Hello,

We have just checked out your Website and the Form Processor File: https://nmaiom.com/contact/include/form.php does not exist on your Server which is causing this issue. We are assuming that the correct location of your Form Processor File is: https://nmaiom.com/include/form.php . So, you will simply need to change action="include/form.php" to action="https://nmaiom.com/include/form.php" in your Forms so that the Form gets processed always from this File.

I am consistently getting a bot check error message when testing the form. Do you know what is causing this? Thank you.
