We have just tried checking your include/form.php File and it appears to be returning a 504: Gateway Time-out error which is a Server Error. So this is not related to Canvas and it is recommended that you contact your Web Hosting Server providers regarding this as they should be able to help you fix this.

Hi Guys, According to my error log I need to include: Date.timezone="Australia/Sydney Or Date default timezone set into the form.php. Where would be the best place?
