We have checked out your Website and the issues you are facing are related to the Customizations made to the Template:

There are several JS Errors on the Page once you submit the Form.
The Main Stylesheet also does not gets loaded: https://elitechimneynj.com/testingcenter/style.css .
Your Customized Website does not use the Default Codes or the Code Structure from the Package.

It is recommended to use the Default Codes to avoid any issues as we cannot guarantee any occurrence of issues due to the Customizations you have made to your Website.

If the Default Codes also do not work, we will fix any issues you are facing with them asap!
