Conditional form Customization

hello,

I'd like to use the conditional form, but there does not seem to be documentation about it.
I want to customize only the field that requires a website url, instead I'd like to ask for a phone number.

Please advise how to do this?

Also: what else needs to be done to make the form work? To which php file should the action point?
