Please find the following code on your Page:

and replace it with your APP ID. Additionally, please find the following code on your Page: 

data-href="http://themes.semicolonweb.com/html/coworker/blog-single-split.php"

and replace it with your Own Website's URL: 

data-href="http://your-website.com/page.html"
