The Medical Niche Demo does not come with a Blank Starting Page by default. Meanwhile, we have assembled a Blank Page for you and attached to this reply.

Do you also have a page showing various form elements such as checkboxes, multiselects etc? Medical only has the contact for and appointment form with limited form field options.

Absolutely! Our Form processor is designed to work with all the Form Fields possible. Just make sure that you follow the Form Fields guidelines mentioned here: http://docs.semicolonweb.com/docs/forms/fields/ and everything else should work perfectly fine.
You just need to add your Form Fields in the HTML File. That's it! Everything else is processed automatically.

And would you please elaborate more on the 'botcheck' feature of the forms? What it is doing, how it is doing, etc. Is it just Javascript based or talking to server too? Because I am not using PHP but Django.

The botcheck Field is used for Honeypot SPAM Protection against Bots. So the include/form.php check if this field is empty or not. You can check more about how Honey SPAM Protection works here: https://www.thryv.com/blog/honeypot-technique/ .
