Autoresponder not working

Hi - i have a contact form here: https://highpointfin.com/contact-high-point-financial.html

when i try to test it and click to submit, it shows the loading icons spinning but never does anything else.

I added the contact info in the include/sendemail-autoresponder.php and have that on the contact page: <form class="nobottommargin" id="template-contactform" name="template-contactform" action="include/sendemail-autoresponder.php" method="post">

i included a screenshot of what i have on the server ... is that correct?

Any ideas? I can't figure it out....thanks for any help you can provide!
