We have checked this out and the codes you are using for your Forms are incorrect. Please enclose your Form within the .form-widget DIV. Please consider checking the Form Setup Documentation for complete instructions: http://docs.semicolonweb.com/docs/forms/form-setup/ . Make sure you add the DIVs as required and this should fix the issue by itself. This should be your default Form Structure:
[ch_pre]
...[/ch_pre]
